I often tell patients: “When you’re going for a walk, just go for a walk.” Don’t take your worries or anxiety with you—leave them behind.

Of course, your mind may still wander. It might pull you back to work stress or home responsibilities. That’s okay. Just notice it, and gently shift your attention back to the present moment.

Being mindful means grounding yourself in the here and now using your senses—what you see, hear, smell, and feel. The sunlight, the grass under your feet, the rhythm of your breath—that’s the present. That’s where peace lives.

Mindfulness isn’t about pushing thoughts away; it’s about choosing where to place your attention. And when we practice this consistently, it can reduce anxiety, increase happiness, and help us feel more in control of our lives.

So today, go for a walk—and just walk.

Living with a chronic illness is hard—really hard.
It’s not just about managing symptoms; it’s about managing everything else that comes with it: the emotions, the routines, the burnout.

Struggling with medical adherence? You’re not alone. Stress, frustration, grief, or feeling overwhelmed can all get in the way of sticking to treatment plans.

In my previous position at New York Presbyterian/Columbia University Medical Center I worked with many children, adolescents and parents on navigating CMI. Many of my patients struggled with adhering to their diabetes care or following through with medical recommendations. At times, their non adherence became life-threatening, which prompted their medical team to pursue medically-informed mental health providers.

As a clinical psychologist, I want to be able to consult with your broader medical team to give you the best possible care. If you or your loved one is struggling with navigating this new world post-news of a CMI, you’re not alone and it’s okay if you don’t have the answers.

I use a compassionate, skills-based approach grounded in Dialectical Behavior Therapy (DBT) to support children, teens, and families facing the emotional and behavioral challenges of chronic illness. While DBT was initially developed for individuals with borderline personality disorder (BPD), it’s been recognized that the skills taught in DBT, such as emotional regulation, distress tolerance, interpersonal effectiveness, and mindfulness, are valuable for a wider population with various mental health conditions and challenges.

DBT skills can be helpful for individuals who struggle with:

Emotional Regulation: Managing intense emotions and impulsive behaviors.

In a world full of distractions and competing demands, the ability to control your attention and stay present is crucial. Many of my patients tell me that this past week, they've struggled to let things go or find themselves worrying about tomorrow. This happens even when the most helpful thing to do is simply put those worries in a box and set them aside for another day. When this comes up, we talk about mindfulness and how powerful it can be for our focus and mental health. After all, if you could control how much your mind focuses on the negative, wouldn’t that make you a little happier?

Building that control takes time and practice. So this week, I encourage you to do one thing at a time. When you're going for a walk, just walk. When you're making coffee, just make coffee. When you're playing with your child, be fully present with them. It’s not easy, but I promise, it gets easier with practice.

Borderline Personality Disorder (BPD) is a mental health condition that impacts your relationships, your sense of self and overall life stability. The above symptoms often start in adolescence, BUT many people don’t get diagnosed until adulthood! There are so many reasons why this happens, including the unfortunate truth that many providers hesitate to diagnose BPD in the younger years.

Did you know that in order to obtain DBT-LBC certification, you have to go through rigorous training, supervised practice, and a comprehensive exam? Receiving DBT-LBC certification is a distinction in the field that the provider you’re working with has the knowledge and skills to provide DBT treatment as it was intended.

In fact, as of January 2025, there are only 37 certified clinicians in NY, 18 in FL and only 10 in CT!

Dr. Marsha Linehan, the creator of DBT, understood that in order to build a life worth living, you NEED to be able to manage distress. Being able to tolerate painful events and emotions is so important in order to get long-term rewards and benefits. I often ask my teens to think about all the things they had to tolerate leading up to skills group. It could be things like: getting a failing test grade, having a hard conversation with parents, or perhaps dealing with the consequences of their behavior after getting caught stealing. Do you know what these examples have in common? They all require you to sit with difficult emotions without doing something that could make it worse.

During the distress tolerance module, you learn crisis survival skills to help get you through the moment. That’s the goal. Get. through. the. moment.

The Walking the Middle Path module is a bonus module for the adolescent/multifamily DBT skills group. It is a module specific to teens and parents to help address differing perspectives and the polarization that occurs as a result.

Teens and parents often enjoy this module because it brings up different ways other families have solved problems that are important to them, such as curfew or a phone privileges. Group provides all families the venue to discuss and consider creative ways in which they might “find the middle path.”

The ‘Middle Path’ refers to finding a balance and path between two opposing extremes — a dialectic. If you ever find yourself feeling stuck with your teen or parent with issues related to parenting, relationships or academic performance — walking the middle path skills are for you.

When you think about your relationships, how do you think you’re doing with balancing all the competing demands that come up? Have you ever noticed how your mind wonders ? You can be driving, but your mind is stuck on something that happened yesterday or worrying about work/school. You might even arrive at your destination and think “whoa- how did I get here ?!?” When you have that experience it’s often because you go on auto-pilot— this happens a lot with behavior that is repetitive or familiar.

Mindfulness is the opposite of autopilot — it is an active practice. It’s work. It involves the ability to pull back from the distracting thoughts so you can live in the here and now. In DBT, we believe that in order to be able to build a life worth living, you NEED to be able to use your mindfulness skills.

Ever feel like your emotions are hijacking you? Or that your emotions are so strong it’s hard to be effective?

DBT skills group are focused on enhancing your capabilities by teaching you skills in a group format. The adult DBT skills group runs through the following modules: Mindfulness/orientation, Distress Tolerance, Interpersonal Effectiveness, and Emotion Regulation.

Each of these modules have their own set of skills. For example, the distress tolerance module is all about how to tolerate difficult emotions without making a situation worse. If you ever feel like it’s hard to resist an urge that you know will only hurt you long-term (e.g., lashing out at others, binge eating, self harm behavior)— these skills are for you.

In skills group, you’ll get an opportunity to learn skills alongside other individuals who struggle with similar experiences in a nonjudgmental setting. I hope to see you there!
